Support for Carers

We welcome the Carers of our patients at Heywood Family Practice. We would like to work in partnership with you.

Definition of a Carer: Individual irrespective of age, who provides or supervises a substantial amount of care on a regular basis of a child, relative, partner or neighbour who is unable to manage on their own due to illness, disability, frailty, mental distress or impairment.

If you are a carer, and haven’t already let us know please complete a Tell Your GP You Are A Carer form  (available from the surgery) and return to Wendy Middlestorb, Carer’s Champion, to receive a Carers Pack.
